    PA form has been signed by both the Dean of the college and the direct supervisor of the student.
     
  Student has a valid social security number. International students may not apply with a 900 social security number. These take a while to process; therefore, please check with your students well in advance.
       
  Please confirm the student’s GPA prior to submitting a PA. This will enable you to prepare a written request of a waiver of GPA requirement (if you wish to make that request). A waiver of the GPA requirement is necessary for any graduate student who has a GPA below 3.0. If at all possible, colleges and departments should give preference to students who meet MSU’s requirements.
       
  If a student resides out of state, particularly internationally, please mark “out of state” on the PA. All graduate assistants receive in-state tuition. The marking of “out of state” allows us to know which students need adjustments to their bills.
       
  Please check the number of hours a student is working (adjusted work hours) and add that number to the number of course hours in which they are enrolled. For example, a student who is taking 12 hours may not work 20 hours. The formula for assessing this involves the number of work hours divided by 5, in addition to number of course hours. The number of work hours divided by 5 is the adjusted work load. Added to the number of course hours, this number cannot exceed 15. If a student is working 20 hours, their adjusted work load is 4. If a student is taking 12 hours, this equals 16. This student has exceeded their amount and must either reduce their course load or work hours. Exceptions to this policy can only be made through the approval of an overload process (form online).   Please make sure the source of funding is clearly communicated on the PA form. If a student is receiving money from the Provost’s office, please indicate that in the “remarks” section. A student may be receiving a tuition scholarship, a 60/40 matching, or another award determined by the University Coordinator of Graduate Studies.
       
  A student must be enrolled in at least 6 hours in order to be eligible to hold an assistantship. A student must be registered before the PA will be processed.
     
  In the event of a resignation, a new PA must be submitted indicating this action and the reason for the resignation. Please be sure the line for resignation is clearly identified.
     
  If at all possible, PAs should be hand delivered to the Provost Office.
